Ceiling tiles are replaced weekly due to water leak. Work order on file. Affix metal strip on carpet in media center. Observed damaged frayed carpet in media center. Observed hole in ceiling tile in girsls toilet in gym; damaged and stained ceiling tile in boys toilet. Observed damaged floor tile in MC240; damaged and stained ceiling tile in RR610. |

Replace missing panel on AC units in room 24 & room 11. Room 24 smells damp and mildewy. Ceiling damaged by condensation in room 24. |

Observed bulk amounts of hand sanitizer, Clorox disinfectant wipes, cleaners, stored on counter and unlocked closet and accessible to the students in multiple classes: rooms. Ensure all hazardous items are stored out of reach of the children. |
