Observed the sink fixtures (sink drains), and pipe covers on he wall were rusty on the outside restrooms.

Observed the walls in the gym in need of cleaning, and the baseboard missing/damaged.
Observed several displaced ceiling tiles in the gym (REPEAT).
Observed the stall partitions with chipping paint in the outside restrooms, the floor was not clean in room M4, and the floor was damaged in MC1197.

REPEAT NON-COMPLIANCE - Observed disinfectant wipes, disinfectrant aerosols and other hazardous items were stored on table tops, under sink, and in unlocked cabinets in several rooms/areas (A11, B32, A19, C40, A6, A8, M4, and the health room) Keep all hazardous products stored as instructed on product labels.
**Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). |
