A - Repeat Non-Compliance: Observed the stall partition was broken and splintered in the girls R6 mobile restroom, the stall partition wall not finished with paint in the boys R6 restroom, **Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).B - Observed toilet seats were loose in the girls 500 hall restroom, toilet paper dispensers were broken in 100 hall girls restroom, sanitary boxes had rust, toilets were clogged in RR657 girls and 400 hall girls restrooms, and the toilet seats were worn/discolored in 400 hall boys restroom. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).C - General Comment: Observed the floor was damaged (broken/lifting tiles) in the boys R6, and a transition strip was lifting in the girls RR657 restroom. **Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). |

A - Observed a faucet knobs were missing (100 hall staff restroom, RR657 men's staff restroom), the sinks were rusty in RR657 restrooms, sinks were not working (RR657 boys, R6 girls), and several sinks with push faucets had short run times less than 125 seconds. **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).B - General Comment - Observed no paper towels available at the boys handicap restroom on 100 hall. **Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c). |

Observed several ceiling leaks/water damage throughout the facility (outside room 114A, outside room 204, 200 hall near breeze way, media center (mold), 200 hall at media center entrance/mall); ceiling panels were missing/displaced in the 200 hall/mall boys restroom, wall with chipping paint in MC228, the carpet was stained in the computer room/media center, the walls were dirty in room 507, and the floor was uneven/soft in the weight room near exercise room entrance.**Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. |
