Observed water stained ceiling tiles and ceiling tiles with microbial growth present in G, B, C D and outside modular units. Repair any and all roof leaks, then replace water stained ceiling tiles. Observed gaps along base of exit door to different building. Provide door sweeps on base of exit doors to eliminate gaps along base of doors. Observed damaged floor tiles throughout in modular units. Repair/replace damaged flooring/floor tiles in outside modular units. Wipe/clean walls throughout in classroom and hallway area. Touch-up paint/repaint throughout in classrooms and hallway area.

Repair damaged flooring in boys dressing room. Thoroughly clean hand toileting, hand washing and shower fixtures in both girls and boys bathroom. Repair damaged bench in boys dressing room. Remove pads from shower in dressing rooms. Repair light fixtures in dressing rooms. Paint bare wood stall fixtures. Damaged stall doors had been replaced but doors were not painted.
