General Comments

General Comment: #7. Observed some storage on the floor in storage room but the floor in storage rooms were clean. Recommend keeping storage off the floor by providing additional shelving to store extra stock/supplies on.**All. storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411. #11. Observed damaged/loose sewage clean out covering between G and B building. Repair and secure clean out covering. Observed hole at base of gutter in corner near B building offices. Fill in hole at base of gutter and place a diverter at base. Reattach loose/missing gutter on modular unit located behind D building. Secure siding on modular restroom located beside cafeteria. There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). Mr. Lonnie and Ms. Shirley present at time of inspection.

OBSERVED LEAK BEHIND/UNDER TOILET IN MEN BATHROOM LOCATED IN B BUILDING OFFICE AREA. WHEN TOILET IS FLUSH WATER IS LEAKING FROM BASE AND CORNER OF WALL! Observed stall fixtures soiled throughout in bathrooms. Observed damaged stall fixture in D building bathroom. Clean toilets, stalls, walls and floor in bathrooms! Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).
5	35	LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409)	No	No	No	Observed hand washing sink soiled around faucet fixtures in bathrooms throughout in both girls and boys bathrooms. Observed several hand washing sink in classrooms missing soap and paper towels. Thoroughly clean hand washing sink around faucet fixtures and keep soap and paper towels at all hand washing sinks at all times! Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).

Observed damaged laminate on book display in media center.
