A: Observed a ceiling tile was missing in 100 hall girls and 400 hall boys restrooms, and the ceiling was splattered with residue in the 100 hall boys restroom. Observed the stall door was broken in the boys 400 hall restroom, and the door and door frame had mold growth in RR658 boys. **Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).B: Observed restrooms RR1155 with no water. Water has been turned off since January 2014. The toilets and urinals were dirty in RR1155. Observed a toilet seat was broken in 600 hall girls restroom, and the toilets were leaking onto the floor in 100 hall girls restroom. **Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).C: Observed the floor vinyl was lifting in in RR658 boys and the floor was collecting debris and webs behind the toilets in RR658. **Floors of toilet facilities shall be impervious and kept clean. 15A NCAC 18A .2408 (c). |

A: Observed sinks with missing knobs in the 100 hall girls restroom, sinks with rust in the boys 300 hall restroom, the sinks were dirty in both of the girls gym locker restrooms, 2 of 3 sinks were not working in RR658 girls restroom, and the laminate backsplash in RR1155 was damaged (separated from wood). **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). |

Observed several areas with water stained ceiling tiles (702, 600 hall). Observed the built-in shelves with chipping/peeling paint in rooms 304 and 404. **Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. |
