(REPEAT NON-COMPLIANCE) Observed all of the urinals (3) were out of order and were covered with black bags in the high school boys restroom, the metal flashing in the toilet stall was bent/damaged in the boys 7th grade restroom, a toilet was clogged in the boys locker room restroom, and the toilet fixtures were not clean in the girls and boys 8th grade restrooms (near stairs). Observed the floor with dark colored residue near the floor drain in the girls 8th grade restroom, and the boys restroom floor was not clean in the high school wing. |
