NOTE - install wall mirror or remove sharp metal brace used to mount wall mirror in the boys high school restroom. (REPEAT NON-COMPLIANCE) Observed all of the urinals (3) were out of order and were covered with black bags in the high school boys restroom, the metal flashing in the toilet stall was bent/damaged in the boys 7th grade restroom, a toilet was clogged in the boys locker room restroom, and the toilet fixtures were not clean in the girls and boys 8th grade restrooms (near stairs). Observed the floor with dark colored residue near the floor drain in the girls 8th grade restroom, and the boys restroom floor was not clean in the high school wing. |

Clean all light fixtures to remove and ensure that source of wasps are eliminated. Observed some AC units in the class room with dust and debris in the vents, a light shield not secure in the 6th grade hall leading to the gym, and a light fixture in the boys 8th grade restroom and band hall with brown residue. **All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b). **Lighting and ventilation shall be provided and installed as required by the N.C.
