NO POINTS FOR THE FOLLOWING VIOLATIONS: #4. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed 2 urinals in need of repair in boys restroom on 6th grade hallway. Head Custodian was aware of issue and has already submitted a work order to have toilet repaired. #5. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).Observed 1 hand sink in need of repair in girls restroom on 6th grade hallway. Head Custodian was aware of issue and has already submitted a work order to have sink repaired. *NOTE: Head Custodian Mr. Smith was present during time of inspection. Observed ceiling laminate peeling in girls and boys locker rooms over showers.

Observed boxes of supplies on the floor in storage closet 309. Observed boxes of books on the floor on stage located in cafeteria.
