General Comment: #3. Observed water nozzle soiled on water fountain at entrance to church/ school/lobby. Thoroughly clean water nozzles on water fountain.**Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A. .2406 (b). #7. Observed cases of paper towel dispenser stored on the floor in down stairs storage/supply closet. Store cases of paper towel dispensers on shelving.**All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411. #8. Observed return air ventilation soiled/dusty in classroom and hallway area. Clean return air ventilation covers. Observed some holes ,spots and stains on chairs in classrooms. Thoroughly clean chairs in classroom and replace chairs with holes present.**All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b). #11. Observed some broken glass around dumpster in dumpster corral. Clean grounds around dumpster in dumpster corral. Fill in low area on the play ground under swings and around border surrounding playground equipment.**The premises of the school under control of the principal shall be kept neat and clean at all times. 15A NCAC 18A .2415 (a). There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). ***Locker Rooms(boys and girls) is completely gutted and church is in the process of renovating locker rooms.*** Last day of school was May 20, 2015.
