Repair wall behind toilet as soon as possible. Observed damaged base tile cove moulding in boys bathroom near office area. Observed wall urinals out of order in boys bathroom near media center and out of order toilet in modular unit bathroom near well. Repair all toilets in bathrooms! Observed underside of toilet, interior of toilet and area where toilet bolts to the floor soiled. Thoroughly clean all toilets/urinals. Observed rubber flooring damaged in boys bathroom located in modular unit wing. Repair flooring in bathroom to help eliminate urine smell. Thoroughly clean floors, walls and ceiling in bathrooms. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b).
