NO POINTS FOR THE FOLLOWING VIOLATION: #9.All fixtures shall be kept clean and in good repair. 15A NCAC 18A .2413 (d). Observed shower fixtures in need of detailed cleaning in both girls and boys showers located in locker rooms in gymnasium. Observed markings on shower partitions. Floors, walls, and ceilings of dressing rooms and showers shall be kept clean and in good repair. 15A NCAC 18A .2413 (a). Observed shower tile in need of detailed cleaning in both girls and boys shower rooms. Per conversation with administration children do not utilize showers however per notated rule the showers must still be clean and kept in good repair. *NOTE: Administrator Ms. Miriam Ray was present during time of inspection. Observed several urinals out of order throughout school building (boys 6th grade restroom, mobile unit boys restroom, boys restroom 8th grade hallway). Observed brown staining in all toilets located in girls restroom on 8th grade hallway. EHS advises detailed cleaning be completed to all toilets throughout school building to include inside and outside of toilets.

Observed faucet coming out of sink in boys restroom near classroom 226. Observed detailed cleaning needed to inside of hand sinks throughout school building.

Observed stained ceiling tiles sporadically throughout school building. EHS advises school to ensure no active leak is occurring and to have stained ceiling tiles replaced.
7	10	STORAGE SPACES: Clean 20#; storage off floor 20# (.2411)	No	No	No	All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411. Observed boxes of supplies stored on the floor in teacher work rooms throughout school building. EHS advises school to arrange storage ordering to notated rule.
