Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d). Observed all toilet bowls with brown/orange ring build up inside located in girls upper level restroom closest to parking lot. Per conversation with Head Custodian the water that runs into that specific restroom is hard and leaves a residual staining in the toilet bowls. |

Observed stained ceiling in upper girls restroom closest to parking lot. Observed chipping floor tile in boys locker room located in gymnasium. Observed active ceiling leak outside of Coach's office in gymnasium building. Per conversation with Head Custodian a work order has been submitted for active leak. |
