A: Observed the laminate sin counter was damaged in boys RR800, a wall mounted soap dispenser was broken in 200 hall girls, and some restroom sinks faucets were no functioning properly (push faucet run for less than 15 seconds. **Adjust fixtures so that the water runs for at least 15 seconds. **Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b).B: Observed no paper towels in boys RR800, and boy 200 hall restroom had no soap. **Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c). |

Observed the floors in the boys and girls locker room were not clean and in need of re-painting, the wall was dirty and the door frame with flaking paint in room 401 (6th grade), the barrier wall/ramp between building with chipping paint, the media center carpet with dark stains, a ceiling tile with mold in the Dance room, water damaged (wet)ceiling tiles thoughout the campus (rm 107, gym hall, 100 hall, dance room)**Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. |
