Observed damaged hand sink counter and cabinet in hallway located in hallway and in residents room. Repair hand sink counter and cabinet. Cabinet area under hand sink is also damaged. I would recommend just replacing hand sink counter and cabinets in bathroom. Observed hand sink in bathroom missing paper towels. Provide soap and paper towels at all hand sink at all times! Each resident will be provided soap and individual towels. These towels will be stored separately after being used. Tightly secure toilet seat in bathroom located within resident room. 15A NCAC 18A .1610 (b).
