Observed damaged shelving under kitchen two compartment sink, damaged and missing drawer and cabinet doors. Replace damaged shelving under two compartment sink. Remove rotten/damaged boards, then place new board down. Repair cabinet and drawer doors throughout kitchen. Observed utensil/equipment storage areas soiled. Thoroughly clean utensil shelving and base of utensil/equipment drawers in kitchen. Clean utensil/equipment shall be stored in or on clean area and in a manner to prevent contamination. Replace utensil shelving which particle board is breaking and flaking onto other shelving.
All equipment and utensils shall be so constructed as to be easily cleaned and shall be kept in good repair. 15A NCAC 18A .1618 (a).

Observed base of drawers in bathrooms soiled. Repair cabinet doors and clean base of drawers. PROVIDE SOAP AND PAPER TOWELS AT ALL HAND WASHING SINKS AT ALL TIMES. Today no paper towels at hand washing sinks throughout house.Clean area around lid of clothes washer machine. Each resident will be provided soap and individual towels.
