Observed edge of up-right freezer door and exterior of cabinet doors soiled in kitchen area. Thoroughly clean edge of freezer door and exterior of cabinet doors. Repair leak under kitchen two compartment sink. Observed leak on plumbing fixutre. Observed under counter equipment/equipment storage area and base drawer under stove where sheet pans are stored soiled. Thoroughly clean shelving and areas where clean utensil/equipment is stored and keep clean at all times! Non-food-contact surfaces of equipment shall be cleaned at such intervals as to keep them in a clean and sanitary condition. 15A NCAC 18A .1618 (c ). PROVIDE SOAP AND PAPER TOWELS AT ALL HAND WASHING SINKS AT ALL TIMES! Observed paper towels missing from several hand washing sinks throughout residents and no soap at hand washing sink in med closet. Observed broken toilet seat in bathrooms. Replace broken toilet seats throughout facility. Observed slow draining hand washing sinks in several bathrooms. Address slow drain hand washing sinks in bathrooms. Observed shower curtain and shower chair soiled in bathroom. Thoroughly clean microbial growth from shower chair and cracks and crevices of chair. Observed base of shower soiled. Thoroughly clean flooring in tile shower. Regrout top of bathtub where tub basin meet wall tiles. Observed grout missing. Clean area around lid to washer machine. Repair or replace broken dryer. Each resident will be provided soap and individual towels. These towels will be stored separately after being used. 15A NCAC 18A .1610 (b). Repair or replace damaged flooring in kitchen area and near dining room/living room area. Clean floors and base boards throughout house. REPAIR FLOOR NEAR ENTRANCE TO HALF BATHROOM NEAR SIDE ENTRANCE DOOR. FLOORING IS ACTUALLY GIVING WAY/ SAGGING! Clean stained carpet in residents room or replace stained carpet in rooms. Repair floor in resident room near closet area.
All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. |

Clean walls and ceiling throughout kitchen, laundry room, residents rooms and bathrooms! Repair roof leak in bathroom. Observed black microbial growth present on ceiling and sign of roof leak because ceiling is also damaged over shower area. Patch holes in walls in residents room and repaint/touch-up paint throughout.
The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. |

Observed old rotten boards stored under deck at end of house. Remove old rotten boards from the premises. Tightly secure loose and missing boards and railing on deck. Store containers(cigarette bucket) so it doen not hold or collect water--> drill small holes in base of bucket/containers so water will drain. The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a).
General Comment: #9. Replace dining room table showing wear/ damaged around edge of table top. Replace pillow covering/case with stains present in resident room. All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a). Clean bed linen in good repair shall be provided for each resident and shall be changed when soiled. 15A NCAC 18A .1617 (b). |
