Observed shower chair and covering on shower chair soiled. Thoroughly clean shower chair and covering on chair. Observed hand washing sinks throughout house missing paper towels. Provide soap and paper towels at all hand washing sinks at all times! Clean area around door area on washer machine. Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). Each resident will be provided soap and individual towels. Observed crawl space door opened under house. Keep crawl space door closed at all times. Observed old rotten boards stored under deck at end of house. Remove rotten boards from premises. The premises shall be kept neat, clean, adequately drained, and free of litter and vermin harborage. 15A NCAC 18A .1615(a). GENERAL COMMENT: #4. Observed base shelving in refrigeration unit cracked/damaged.
