Observed base of utensil/equipment drawer under oven/stove soiled. Thoroughly clean base of drawer under oven/stove. Utensils shall be handled in such a manner as to prevent contamination. 15A NCAC 18A .1618 (f).General Comment: Clean ice from top of door of up-right freezer unit. Monitor freezer unit. Ice should not be forming around top of door. Gasket on refrigeration unit is in good repair. Make sure items in freezer unit are properly stored so they not prevent the freezer door from properly closing. |

Recommend installing transitional floor moulding at entrance to residents room. Observed edge of carpet starting to fray at entrance to resident rooms.
