General Comment: Stained/ etched clear plastic cups discarded at time of inspection. Opened bag of corn meal placed in sealable container at time of inspection. Tightly secure loose transitional floor moulding between dining room area and living room. Repair flooring near laundry room. Recommend providing a wider transitional floor moulding near laundry room. Thoroughly shampoo carpet throughout in all rooms, living room and exercise/activity room. Observed stains throughout on carpet.All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607. |

Observed wall damaged behind bathroom door. Repair wall behind bathroom door. Observed loose handle on resident bedroom door. Repair loose handle on door. Observed gap along base of wall and flooring in resident room close to laundry room. Gap should not be along base of wall and flooring. Have someone to check foundation of house to see if house is settling. Also observed hair line cracks on ceiling in hallway and sheet rock tape coming loose from edge of ceiling in exercise/activity room. Repair cracks along ceiling and repair ceiling in activity/exercise room. Observed water stains on ceiling in activity/exercise room. Have someone to check ceiling for any roof leaks and repair if needed. The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608. |

Observed light fixture not properly working in bathroom above hand washing sink near laundry room. Observed broken/damaged window blinds throughout rooms/residents room. Replace broken window blinds throughout. Observed ceiling fan blades soiled/dusty. Clean ceiling fan blades. All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a).
