Observed torn/damaged dining room chairs. If stains does not come off of tub/shower and walls, facility might want to have tub/shower and walls resurfaced/reglazed! Thoroughly clean hand rails in shower/tub.Toilet, hand washing, bathing, and laundry facilities shall be kept clean and in good repair. 15A NCAC 18A .1610 (a). |

Observed carpet stained throughout in hallways and residents room. Shampoo carpet throughout facility where needed. Observed loose edges of flooring in bathrooms near toilets. Tightly secure flooring in bathrooms! Observed gap along base of wall and flooring in resident room located near the laundry room. Replace missing shoe moulding along base of flooring and wall. Clean walls around/near toilets |
