21. Observed toilet seat damaged in men's staff restroom. Observed cabinet under diaper changing table collecting debris, wood damaged from water.Observed wet residue on front of toilet in Pre3/Pre4 boys classroom restroom. Recommend checking to ensure wax seal is in good condition.Caregiver state incorrect change of use in Pre 3 classroom. Keep toilets and urinals clean and in good repair. 15A NCAC 18A .2817 (a). Keep handwashing sinks clean and in good repair. 15A NCAC 18A .2818 (a).
