Patricia Pharrs was present during the inspection.#29 - (CRITICAL) Observed a white-out pen was on the floor in the care room. **Keep all hazardous items stored behind lock and key.#37 - Observed a floor tile was broken and not secure to the floor in the restroom; part of the sub-floor was exposed. **Keep all flooring in good repair; flooring in the restroom must be easily cleanable and non-absorbant.Observed the exterior door frame at the exit door with chipping paint. **Keep all painted surfaces intact.
