**General Comments** 27. Ensure that diaper changing stations are free of all storage. Observed parents placing items on diaper changing tables without teachers and childrens being present in the room. Please remind parents not to store items on diaper changing tables. 34. Observed one missing pin on art easle in Davidson room, observed stained area rug in davidson room. Ensure that all fixtures are in good repair and floors/carpets are cleaned. 37. Observed water puddle on floor near wash-machine in laundry room. Remove water from floor and ensure fixtures are not leaking. Touch up paint on walls where needed. 39. Remove dust buildup from ventalation throughout. Also observed blown light bulb under hood systems in kitchen. Ensure that 50ftcd of light is provided while cooking. Replace bulbs as needed. 25. Provide properly mixed disinfectant at all times so that diaper changing tables can be properly cleaned and disinfectanted.

At arrival of inspection observed disinfectant too weak in infant 1 and infant 2 rooms. Assistant director pulled bottles and properly remade disinfectant throughout center so that is is 500-800 ppm -2 points.
