Keep all floors, walls, and ceilings, clean and in good repair. 15A NCAC 18A .2824 and .2825. Observed minor stained ceiling tiles sporadically throughout center. EHS advises center to ensure no active leak is occurring and to have stained ceiling tiles replaced.
