Observed teacher handle child in the middle of diaper changing process. Teacher completed the child portion of diapering and handled a crying child before washing hands. Teacher then finished the cleaning and disinfecting portion and washed hands at end. Must remember to wash hands before handling a chlid after diapering, even if process is not complete.
Wash hands when required. 15A NCAC 18A .2803 (a) Repeat violation. |

Observed black residue on vent in kitchen area of 3 year old room and dust on vent in 3 year of class above centers. Repeat. Observed dust on vent in ladies room near infants class.
