NO POINTS DEDUCTED #5 Observed baby bottle not labeled from brought from home. **Label all baby bottles, sippy-cups, and bottled beverages sent from home with the child's name and date it was brought into the facility. 15A NCAC 18A .2804 (d) #21 Observed leak at hand sink nozzle in 2's restroom. Leak in need of repair. #23 Provide paper towels in adult hall restroom near toddler room. No paper towels present during inspection. Towels provided during inspection. Provided hand washing sing at hand sink in vacant 4/5's class room. Hand washing sign attached to email. #27 Observed caregive switch steps when wiping hand with moist towel after wiping childs hands with moist towel. Follow correct diaper change procedure. Observed paper towels stored in plastic container on top of diaper changing surface. Keep diaper changing surface free of storage. #37 Observed black mold present on ceiling in laundry room. Keep floors, walls, and ceilings, clean and in good repair. Ms. Polk was present during inspection.
