Observed diaper changing table in infant room with damaged siding and tape present on siding. Properly repair siding on diaper changing table or replace damaged siding.**Diaper changing surface must be easily cleanable, non-absorbent, smooth, and intact. 15A NCAC 18A .2819 (b). Observed disinfectant bottles not properly labeled. Label disinfectant bottles "disinfectant". Today bottle in toddler room and pre k 3 room labeled 500ppm. Re-label fading marking on sanitizer, soapy water and disinfectant bottles.**All cleaning and disinfectant solutions shall be properly labeled (i.e. soapy water, disinfectant). 15A NCAC 18A .2819 (c ) and .2820 (c ). |

Observed soiled handles and underside of tables throughout different classrooms in day care. Observed protruding screw on toy fixture/desk in 3 year old room and pre k 2 room. Observed soiled linen on pad in play center in 3 year old room. Observed soiled counter in three year old room. Thoroughly clean chairs, tables and linen on furniture in play center. Remove any protruding screws/bolts on furniture/fixture. Tighten loose screw on furniture/fixture(mirror).**Toys, equipment, furniture, and any other items accessible to the children shall be kept clean and in good repair. 15A NCAC 18A .2822 (a), (b). Highly recommend resealing furniture/fixtures which varnish has came off exposing bare wood surfaces. Remove water from water play table and clean and sanitize are each use. |

Observed water stained ceiling tiles in infant room. Replace water stain ceiling tiles since roof has been replaced. Repair ceiling in toddler room and kitchen area. Repair lower section of wall in toddler room. Repair floor in pre k 3 room under handsink in classroom. Tightly secure loose rubber cove base moulding in pre k 2 room. Clean floors, walls and ceiling throughout where needed. Clean walls in bathroom under paper towel dispenser and around light switches. Patch/seal hole in door in toddler room and pre k 3 room. Highly recommending resealing doors with a wood sealant. Monitor area around window in pre k 2 room and repair if needed.**Keep all floors, walls, and ceilings, clean and in good repair. 15A NCAC 18A .2824 and .2825. Discard and replace rug with frayed corners in 3 year old room. |
