Keep all floors, walls, and ceilings, clean and in good repair. 15A NCAC 18A .2824 and .2825. Observed stained ceiling tile in PreK 1 classroom and outside of After school classroom. EHS advises center to ensure no active leak is occurring and to replace stained ceiling tile. Observed carpet staining throughout center. Per conversation with Director carpet is scheduled to be replaced throughout center soon.
