General Comments

Tonya Littel-Reid (director) was present during the inspection. #5 - Observed opened bulk/commercially packaged soy milk and "lactaid" milk container labeled with 1/24/14 and 2/25/2014 dates. Product label indicates products should be consumed within 7 days of opening. Advised staff to also date the milk containers with the date opened and consume/discard within 7 days. **Label all baby bottles, sippy-cups, and bottled beverages sent from home with the child's name and date it was brought into the facility. 15A NCAC 18A .2804 (d)#37 - Observedseveral ceiling tiles with water damage and painted over with white paint. **Replace all water damaged ceiling tiles. **Keep all floors, walls, and ceilings, clean and in good repair. 15A NCAC 18A .2824 and .2825.#46 - No CCA treated wood present.***Required hot water heater repair (replaced all elements) was completed on 2/26/2014. Documentation of repair was provided.
