#12. Observed build up on top portion of removable plastic holder in baby bottle warmer. Bottle warmer and parts shall be cleaned daily. **Bottle warming equipment shall be cleaned and sanitized at least daily and when visibly soiled. 15A NCAC 18A .2810 (f) (1) and .2804 (d).#32. Observed ripped mat in storage closet in after school room. Mats were not in use. Keep all mats in good condition. **Keep all mats, cots, and cribs clean and in good repair. 15A NCAC 18A .2821 (a).#25. At time of inspection disinfectant was below 500 minimum ppm. Pulled all disinfectant bottles during inspection, demonstrated and prepared correct solutions, which were then distributed to class rooms. If disinfectant is found weak next inspection a provisional classification will be issued due to improper sanitization of diaper changing table. **The diaper changing surface shall be properly disinfected between each child. 15A NCAC 18A .2819 (c ). #29 Observed ice pack and antiseptic hand cleanser both stating keep out of reach of children were found in first-aid kit in hallway leading into 3's room. Hazardous items shall remain behind lock and key. **All hazardous items must be stored behind lock and key. 15A NCAC 18A .2820 (b).#49 Observed 2 wagons on back playground missing side doors. Same plastic cars collecting water on back behind seat.
