Observed one torn diaper changing pad in room 2110. **Diaper changing surface must be easily cleanable, non-absorbent, smooth, and intact. 15A NCAC 18A .2819 (b). 34. Observed one xylophone beginning to rust in room 2112. Director had xylophone removed. **Toys, equipment, furniture, and any other items accessible to the children shall be kept clean and in good repair. 15A NCAC 18A .2822 (a), (b). **Carpet was replaced on 08/15/17.
