General Comments: #3.Keep all walls and ceilings clean and in good repair. See rule .1310.** Observed ceiling leak on first and second floor. Facility maintenance was in the process of addressing leaks. Continue to clean scuff mark from ED patient room 20 and CT/Radiology. #32.**All furniture, bed springs, mattresses, sleeping mats, draperies, curtains, shades, venetian blinds, or other furnishings in institutions shall be kept clean and in good repair. See rule .1319 (a). Observed blood spots on back of lead shields in Cath Lab. Lead shield cleaned at time of inspection.
