1) Keep all flooring in good repair. 15A NCAC 18A .1309 (a) and (b). Observed cracked sloped floor in procedure room 2 in ACU. 2) Keep all flooring clean. 15A NCAC 18A .1309 (a). Observed grout in need of detail cleaning in med surg family waiting bathroom, room 221 bathroom and room 201 bathroom. 3) Keep all walls and ceilings clean and in good repair. 15A NCAC 18A .1310. Observed back splash in mother baby nourishment room separating from wall. REPEAT VIOLATION Significant improvement from previous inspection. 9) Toilet rooms shall not be used for storage. See rule .1312 (b). Observed wheelchair and walkers stored in ICU tub room. 13) Maintain the hot water at all resident accessible handwashing sinks and bathing facilities within the temperature range of 100°F to 116°F. 15A NCAC 18A .1312 (e). Observed hot water below 100F in 2246 of med surge. Water reached 96F and then went back down. Hot water throughout rest of facility was above 100F. 18) Label all disposable water pitchers with the resident's/patient's first name and last name. 15A NCAC 18A .1314 (a). Observed no label on ICU 1 patient water pitcher. 33) Clean bed linen in good repair shall be provided for each individual and shall be changed when soiled. 15A NCAC 18A .1319 (b). Observed stained linen in rooms 237, and CDU 1217. Observed hole in sheet of room 237. 34) Clean linen shall be stored and handled in a separate room or area, or in another manner that will prevent contamination of clean linen. 15A NCAC 18A .1319 (b).
