Observed wall damage in rooms 3104, 5125, and supply room. 8)Toilet, handwashing, and bathing facilities must be conveniently located and readily accessible to residents and staff. 15A NCAC 18A .1312 (a). Observed pink build up on shower head in room 5131. 34) Clean linen shall be stored and handled in a separate room or area, or in another manner that will prevent contamination of clean linen. 15A NCAC 18A .1319 (b). Observed clean linen cart in rehab gym to have a mesh cover, that does not adequately protect the linens.
