Observed chair storage in Rehab 5 restroom. Observed damaged floor drain in 304, 316. Observed worn drain in 3rd floor nourishment room. Observed calcium build up in Emergency Department patient room #12. Staff stated incorrect hand hygiene procedure for hand washing at 6th floor processing of CPAP machines. Staff stated hands would be washed on clean side processing sink.
