General Comments

General Comments: #1. Thoroughly clean flooring under shelving in storage rooms/areas throughout ICU, 6A, 6C, Neuro ICU, CCU, 5A, 5D and 5F. ****Keep all flooring clean. See rule .1309 (a). #3. Observed minor wall scuffing on walls throughout 3A, 4A and 6F, particularly in soiled utility rooms. Touch-up paint throughout rooms in behavioral units. Recaulk around back of toilets in ED. Tightly secure loose wall covering behind hand railing in hallway area. Tightly secure covering on door outside of CT. ****Keep all walls and ceilings clean and in good repair. See rule .1310. #8. Observed discoloration and black residue on corner of shower in room 800. Observed hand washing sink and counter soiled in room 730. Observed leaking toilet in first floor public bathroom---> leaking toile addressed at time of inspection. Much improvement has been made since last inspection. ****Keep all bathing equipment clean and in good repair. See rule .1312 (a) and (f). **Keep all toilet equipment clean and in good repair. See rule .1312 (b). #17.Observed minor build-up on the nozzles of water fountain throughout 3A, 4A and 6F units. Observed high water pressure on water fountain on 5th floor (5D). Adjust water pressure so water does not splash onto the floor. **Drinking fountains shall be kept clean and properly regulated. See rule .1314 (a). #19.Observed pink residue on ice chute of ice machine in ICN (critical) ****Ice machines, buckets, other containers, and scoops shall be cleaned on a regular schedule such that they are kept free of scum, rust, mold or other contamination. See rule .1314 (b). #28. Observed items on the floor in storage rooms under shelving throughout in ICU, 6A, 6C, neuro ICU, CCU, 5A, 5D, 5F. Minimize the amount of items placed in storage bins and on shelving to prevent items from falling onto the floor under shelving. ****All patient care or consumable items shall be stored at least 8 inches above the floor. See rule .1318 (a). #33. Observed tape adhesive on clean sheet. Have CNA's to monitor for tape adhesive on clean linen when changing linen. Observed soiled linen on bed in ED.-----> Linen changed on bed in ED at time of inspection. ****Clean bed linen in good repair shall be provided for each individual and shall be changed when soiled. See rule .1319 (b).
