3. Observed ENDO hallway and ENDO area with damaged walls and doors. Observed wall with residue and damaged wall paper in Mother Baby room 874. Damaged wall at mop sink in PEDS soiled utility near the OR. Damaged door handle at sterile room in OR. Damaged walls observed throughout OR area. Damaged wall bumper at bay 17 in OR recovery. Damaged walls in 353 and 356. Loose baseboard in 366. Keep all floors, walls, and ceilings clean and in good repair. 5. Observed vents in need of cleaning in PEDS 229, 215, throughout radiology, 3C, 4C, 5A, 6A. Keep vents clean and in good repair. 10. Observed bed pan with no label in 5F 5801. Label all bed pans with the patient’s name and date the pan was issued. 12. Observed nourishment rooms in need of hand sanitizer in 5D and 6D. Observed 7D soiled utility room in need of hand sanitizer. Observed no paper towels in ENDO soiled utility room. Maintain soap and disposable towels in at all sinks. In rooms constructed prior to 2003, provide hand sanitizer when sinks are absent. 13. Observed hot water in room D589 at 118 F. Observed hot water at 96 F in PEDS 229 and 344. Maintain hot water between 100 -116F. 17. Observed water fountain near PEDS 210 with residue. Keep all water fountains clean. 24. Observed fruit flies in 3C nourishment. Keep the facility free of pest. 33. Observed sheets with residue in PEDS 229, ENDO bay 26, Room 800 Mother Baby, Labor and Delivery C-Section room 2. Sheet with hole in PEDS 215.
