3. Observed the following: Walls in need of minor repair on 6th and 7th floors. Intensive Behavioral Health isolation room walls with residue. ED care station D with a damaged wall and tape on the wall.Damaged ceiling tiles in NICU near charge nurse desk. Damaged ceiling in Nuclear Medicine room 5. Damaged wall in High Risk OB clean storage room. Keep walls and ceilings clean and in good repair.13. Hot water in Dialysis unit observed at 91-97 F.Maintain hot water between 100-116F.17. Water fountain observed with black residue on PEDS North Hall. Observed water fountains with high water pressure on 7C, 3C, and 6C. Keep water fountains clean and in good repair. 21.Dumpster area observed with loose trash.Keep the dumpster area clean. 32. Observed the following: Bed frames observed with rust in PICU room 1, PEDS room 208, 4C 443. Missing drawer handle in room 5C room 540. Stained mattress in room 5D room 585. Damaged mattress room 852 Labor and Delivery. Residue under mattress in 871 Labor and Delivery. Debris collecting under a mattress in Mother and Baby nursery. Day bed with debris in room 816 Mother and Baby. Keep all furniture in good repair. 33.Observed soiled linen not in a covered container in a PEDS soiled linen room, Labor and Delivery soiled utility, Mother and Baby soiled utility. Stained pillow case in 5A room 524, 7C 754. Keep all soiled linen stored in a covered container. 34.Observed uncovered clean linen stored on a chair in 5F storage room. Keep clean linen covered. 40.Observed food of patients no longer present in 4C nourishment. Observed opened personal milks stored back in the refrigerators in 6C and 6A nourishment rooms. Discard patient food once they are discharged from the hospital. Do not place food that has been partially consumed with unused food items. 43.Observed a damaged thermometer in 5D nourishment room.
