LCH 5 repair holes in wall behind public restroom doors. LCH 8 med room. and treatment room with residue on the wall. 8L APC unit soiled utility, 8T 8918 with a stained ceiling tiles. Restroom ceiling damaged 7A 7113.4. Lights not working in 5T 5922, 5921 restrooms5. Vent with dust 9A-9107, 3T 3911, 3T 3936, LCH 7 room 7054. 12. 6B clean utility, LCH 8 clean supply room missing soap at handwashing sinks.13. 4T, Endo scope room, sterile processing handwashing sinks hot water not reaching 100 F15. Shower wand missing backflow in 9E 9512. 17. Observed water fountains near Med Air, main lobby collecting residue. Observed water fountains near 3T and ED employee entrance with high water pressure causing water to splash onto the floor. 19.
