Observed wall damage inside resident room #472; Observed wall paper peeling in resident rooms throughout. #15 Ensure back flow preventor is installed in areas that require; Observed no back flow preventor inside resident room #209. #30 Ensure that sharps containers are affixed to med carts; Observed all sharps containers stored not affixed to carts (sitting on top). #36 Maintain food contact surfaces / equipment clean and in good repair. Observed coffee machine nozzles in need of cleaning throughout all pantry rooms; Observed refrigerators inside all kitchenette areas soiled in need of cleaning. #39 Proper date marking of food products; Observed salads made in main kitchen and held in kitchenette areas without proper date marking.
