Tubs must be cleaned and disinfected according to manufacturer's instructions. Many have self circulating disinfectant that is not being used by facility. Observed shower chairs in rooms 312 and 329A in need of cleaning. Observed shower heads in rooms 223 and 252 with pink build up. REPEAT VIOLATION

Laundry areas and equipment shall be kept clean. 15A NCAC 18A .1319 (b) and .1312 (a). Observed washing machine gaskets in need of routine cleaning in all neighborhoods. Gaskets had gray build up as well as personal affects from residents.
Clean linen shall be stored and handled in a separate room or area, or in another manner that will prevent contamination of clean linen. 15A NCAC 18A .1319 (b). Observed one piece of clean linen on the floor in pinebluff neighborhood. CDI: Linen removed to soiled area.
