#3 Keep all walls and ceilings clean and in good repair. 15A NCAC 18A .1310. Observed minor scuffs on wall of clean linen closet in laundry room, living room near main entrance and in spa room near 2nd entrance. Facility is currently undergoing renovations to include painting walls and ceilings. Observed door in activity kitchen offset and did not close easily. Staff was notified and stated flooring is undergoing replacement and contractor will fix door.
#32 All furniture, bed springs, mattresses, sleeping mats, draperies, curtains, shades, venetian blinds, or other furnishings in institutions shall be kept clean and in good repair. 15A NCAC 18A .1319 (a). Observed frayed arm on chair in activity kitchen near 2nd entrance and living room near main entrance. Observed minor stains on pillows in rooms 8, 19 and 25. Staff was notified. Discussed with staff to regularly check pillows, notify family and replace as needed.

Observed coffee machine in activity kitchen near nurses station. Clean after each use.
