No Points Deducted: 3. Keep all walls and ceilings clean and in good repair. 15A NCAC 18A .1310. -Observed damaged wallpaper in various areas in the hallway on the second floor. Repair. -Observed stained ceiling tiles on the 2nd floor. Replace. 5. Keep all ventilation equipment clean and in good repair. 15A NCAC 18A .1311 (b). Observed AC filter with heavy debris build-up in Room 1218. Ensure to increase cleaning frequency. 10. Per an employee, she cleans bed pans in the resident's shower. *USE THE HOPPER Where bedpans are cleaned in patient/resident rooms, bedpan cleaning facilities shall consist of a water closet with bedpan lugs or spray arms. 15A NCAC 18A .1312 (c). 21. Observed the dumpster open, one of the doors broken, and a rusted hole in the front. Change unit. All solid wastes containing food scraps or other decomposable materials shall, prior to disposal, be kept in leak-proof, non-absorbent containers, which shall be kept covered with tight fitting lids when filled or stored, or not in continual use. 15A NCAC 18A .1316 (a). 22. Observed trash on the ground along with mattresses and furniture outside by the dumpster. Clean. All solid wastes shall be disposed of with sufficient frequency and in such a manner as to prevent insect breeding or public health nuisances. 15A NCAC 18A .1316 (d). 27. Observed a heavy pet smell in room 1215. Observed cat food and spills around the litter box. Clean. Animal pens, litter boxes, bird cages and other areas on the premises shall be cleaned to minimize accumulation of animal wastes, pet dander and allergens. 15A NCAC 18A .1317 (e).
