General Comments

NO POINTS WERE TAKEN FOR THE FOLLOWING: #2 - Observed cleaning needed on the floor by the toilet in room 131. **Keep all flooring clean. 15A NCAC 18A .1309 (a). #3 - Observed base board damaged by shower in room 131. **Keep all walls and ceilings clean and in good repair. 15A NCAC 18A .1310. #9 - Observed wheel chairs being stored in upstairs spa. **Toilet rooms shall not be used for storage. See rule .1312 (b). #12 - Observed no soap or paper towels present in upstairs med room. Observed soap to come out of dispenser brown and soiled in soiled utility room on first floor. **Provide liquid soap and disposable towels at all hand washing sinks. 15A NCAC 18A .1312 (d). #14 - Observed no disinfectant present in upstairs spa. Disinfectant provided. **A cleaning detergent and an EPA registered hospital disinfectant shall be accessible to all bathing areas. 15A NCAC 18A .1312 (f). #16 - Observed backup water supply plan to state where backup water will be obtained from for all necessary activities but it does not give details on how each process will be carried out. Backup water supply plan needs to have detailed information on how water will be used for food preparation, hand washing, cleaning, bathing, laundry, and dish washing. Plan also needs to state that the health department will be contacted if loss of water exceeds 4 hours. **Observed an incomplete backup water supply plan. Provide a written plan that meets all parts of 15A NCAC 18A .1313 (f). #17 - Observed cleaning needed on water dispenser nozzle of upstairs nourishment area. **Drinking fountains shall be kept clean and properly regulated. 15A NCAC 18A .1314 (a). #34 - Observed clean linen being stored on high shelf in beauty salon. Linen must be covered for protection. EHS suggested using bags. Observed cleaning needed on personal clothing washing machine. **Clean linen shall be stored and handled in a separate room or area, or in another manner that will prevent contamination of clean linen. 15A NCAC 18A .1319 (b). **Laundry areas and equipment shall be kept clean. 15A NCAC 18A .1319 (b) and .1312 (a). #43 - Observed thermometer missing from upstairs nourishment area refrigerator.
