Resident in hospital at time. #30--Observed cups/single use items stored unprotected on med carts. #32--Observed separation curtain stained in 200 hall spa. Facility will start remodeling soon. Was supposed to start on 5/11, but did not due to gas shortage. Covid 19 restrictions, no signature required.
