Observed some slight wall damage and some area of wall needing some touch-up paint. Continue to make necessary repairs to walls throughout in residents room, hallway and janitor closet. Repair wall around base of can wash in janitor closet. Patch hole in wall in room 166 and install door stopper if necessary. Monitor ceiling through facility and make any ceiling repairs if needed. Facility has bided out roof repair/replacment; facility will just have to make final decision Tighten loose door handle to bathroom in room 261. Tightly secure any loose base board molding in janitor closet. Keep all walls and ceilings clean and in good repair. 15A NCAC 18A .1310. |

Observed underside of shower bed padding soiled. Observed broken leg on shower chair in bathroom between 304 and neighboring room. Thoroughly clean underside of padding on shower chair. Repair leg on shower chair or replace. Continue to replace pitted faucet fixtures on hand washing faucet throughout in bathrooms. Maintenance man (Mr. Kevin) has been replacing faucet fixtures throughout in rooms. Observed grout missing from between floor tiles in shower area in shower south. Regrout floor tiles in shower!
