#2 Observed carpet stains present in spotted areas of facility. Carpets shall be kept clean and odor free. 15A NCAC 18A .1309(a)#5 Observed 1 A/C vent collecting debris in room 123. All other vents were in good repair. Clean vent in room 123 and continue to keep vents in good repair. 15A NCAC 18A .1311(b)#15 Observed 1 shower wand missing back flow preventer in North Spa. Provide back flow prevention device. Improvement seen as to all other shower wands present in spa and resident rooms containing back flow prevention devices. 15A NCAC 18A .1313(d)#24 Observed 1 ant present in South electrical storage closet. Continue to utilize pest control company and keep property free of insects and vermin. 15A NCAC 18A .1317(a)#30 Observed 1 soiled pill crusher on med cart on West 2. Pill crushers shall be cleaned regularly and when visibly soiled. 15A NCAC 18A .1318(c)#36 Observed South resident refreshment room collecting debris in cabinets as well as in bottom of fridge.
