General Comments

#3 Keep all walls and ceilings in good repair 15 NCAC 18A .1312. Observed damaged ceiling in the resident storage area. Observed water stained ceilings on the ceiling tiles around the vent in OBT laundry room. REPEAT #5 Ventilation equipment shall be kept clean and in good repair. 15A NCAC 18A .1311. Observed dust build up on several air vents in resident rooms. A maintenence order has been issued to replace air vent filters on 11/21 and is in progress. #8 All toilet, handwashing, and bathing facilities shall be kept clean and in good repair. 15 NCAC 18A .1312 (a). Observed a loose toilet seat in room 109. Observed a soiled chair in spa #3. #16 The back up water supply plan shall include a plan for an alternative source of water for essential functions such as food preparation, hand washing, bathing, cleaning, dishwashing, laundry, and disposal of bodily waste. 15 NCAC 18A .1313 (f). Observed in the back up water supply that culinary and nursing staff will be provided hand sanitizer for handwashing. These procedures should be changed to use potable for handwashing. There should be more information should be provided on who will deliver the bottled water and the laundry procedures within the four hour time frame.
