Observed soiled toilet chair, need detail cleaning and damaged toilet seat, need repair/replace in AL room 350. Damaged toilet seat in room 252, need repair/replace. Observed hand sink in room 365 is not draining, need repair. Observed leaking shower head handle in spa at healthcare, need repair. |

Observed (at AL) a rolling rack of clothing, wheelchairs, walkers, trash bin, brooms stored in ladies and men’s restrooms. Items was taken out during inspection. Restrooms doors taped and blocked out as it supposed to be out of order and under construction.
.1312 (b) A sign shall be posted in each toilet room for institution employees, directing them to wash their hands after each visit to the toilet room and before returning to work. Observed no hand washing signs in the restroom where employees have access to. Hand washing signed posted during inspection. |
