3. -Observed discoloration (what appears to be mold) under the window, on the walls, and around the outlet on the high numbered end of the 2nd floor hall.-Observed an active leak above the ice and vending machine on the 2nd floor. The ceiling is damaged and bulging. Floors, walls, and ceilings located in Lobby, Halls, Stairs, and Vending Areas, shall be kept clean and in good repair. 15A NCAC 18A .1808 (b).17. Observed black buildup inside of the ice machine on the 4th floor.Ice Machines, equipment, utensils, and the room or area in which the machines are located shall be kept clean and in good repair. 15A NCAC 18A .1811 (c).27.
